When AI Fails, It’s Usually a Leadership Problem

AI failures rarely come from flawed models. They come from misaligned mandates. Projects are approved without a clear operational owner, business units are expected to “adopt” systems they didn’t design, and leadership moves on after funding the pilot.


This isn’t a tooling issue — it’s a strategy execution gap. AI is transformative only when it rewires how the business runs. That requires leaders to do more than green-light the work. They need to architect the conditions for success.


The key failure mode is diffusion of responsibility. AI becomes someone else’s job — usually the data team’s — while core functions maintain status quo processes. Governance is either overbearing or non-existent. Metrics are vague, incentives misaligned, and accountability optional.
